Free medical insurance eligibility


ATTENTION: Pregnant Women and Parents of Children who may be eligible for services. To access these programs, please follow one of the following options:

1) To check potential eligibility yourself, press the "Search for Help" box located at the top of this page and follow these instructions:
Step 1. Click link "Search for Help" to go to the Community website.

Step 2. Scroll down and Click on Find Help

Step 3. Scroll down and Click on Advanced Search

Step 4. Scroll down and click on "Check Eligibility" Step 5. Follow the directions on the eligibility screens please ensure you answer all questions, then click on the Next button.
2) To complete an application for the state medical insurance programs, please press the appropriate program listed above, fill it out online or print out a copy of the application from your computer and fill it out. (This is an Adobe Acrobat form. If you do not have Adobe Acrobat you can download it for free from Adobe Website) When you are ready to mail in your application, please see the appropriate addresses below.
